Team-Based Care

We provide care through a team-based model. This means your care will be provided by your Nurse Practitioner as well as different members of our healthcare team depending on your needs.

Switching Nurse Practitioners

Patients are assigned to a Nurse Practitioner based on clinic processes, availability, and capacity. Requests to switch Nurse Practitioners are not permitted or accommodated.

Code of Conduct

We are committed to maintaining a safe, professional, and respectful environment for patients, families, visitors, and staff. We understand that health concerns and navigating the healthcare system can be stressful. Our team is here to provide support, guidance, and care with compassion and respect. In return, we ask that patients and visitors treat our staff and other patients respectfully. Disrespectful, abusive, threatening, or harassing behaviour will not be tolerated. This includes inappropriate language, intimidation, threats, or refusing to follow reasonable clinic directions. Thank you for helping us keep the clinic a safe and welcoming space for everyone.

Appointment Cancellations and Missed Appointments

To help us provide timely care to all patients, please let us know as soon as possible if you need to cancel or reschedule an appointment. Whenever possible, we ask for at least 24-48 hours’ notice so the appointment can be offered to another patient.

Controlled Substances

Narcotics and other medications that may carry a risk of dependence are not prescribed at a first visit. These medications will only be considered after a full assessment, review of relevant medical records, and a discussion about the ongoing need, risks, benefits, and safer alternatives.

Prescription Renewals

Please allow 1–2 weeks for prescription renewals. For routine renewals, please contact your pharmacy and ask them to fax us a prescription renewal request. This helps ensure we receive the correct medication information.

Forms

Most forms require an appointment before they can be completed. Please allow 2–4 weeks’ notice for form review and completion. This gives the provider enough time to review your chart and complete the form appropriately. Some forms are not covered by OHIP and may have an applicable fee.
