What is Best Care?
Best Care is a complete, guideline knowledge translation module that utilizes team care with a Certified Respiratory Educator (CRE) and a primary care physician or nurse practitioner to deliver all of the elements of evidence-based care defined by provincial, national, and international guideline standards.
Our program is a care delivery module including standardized high-impact best-practices, an interdisciplinary care model, an electronic care delivery and evaluation system, an effective community collaboration model, proven clinical effectiveness, with a demonstrated spread and scale strategy. The program is proven to dramatically improve patient health outcomes while achieving substantial reductions in acute health services.
